Besides its two founding editors, the first edition had 55 contributors, compared with 217 contributors in the tenth edition. The first edition was written almost entirely by authors based in the United States, whereas authors from 15 countries have contributed to the pages of the tenth edition. The vast majority of chapters in the first edition were written by a single author, whereas most chapters now have two authors.
